The LPCHD does childhood Immunizations for infants and children ages 0-18yrs of age by appointment only. We follow the Advisory Committee on Immunization Practices (ACIP) recommendation for administering immunizations. A schedule of when the immunizations are administered can be found Here.

Indiana Vaccine School Requirements.
We can offer immunizations to the following individuals:
- Medicaid recipients
- Uninsured
- Underinsured (has health insurance that does not cover immunizations)
- Insured
If you have health insurance that covers vaccines please contact them prior to your appointment to verify coverage and copay.
Please come prepared. You must present a Medicaid card or Insurance card at the time of appointment for services to be rendered.

“Sexually Transmitted Disease Data Building Tool”- The NCHHSTP Atlas Plus is a tool that gives the user the ability to create tables, maps, and charts using more than 15 years of HIV, viral hepatitis, STD, and TB surveillance data that are reported to CDC. Users can also download the data to do additional analyses. The AtlasPlus has Charts, Maps, and Tables functions. The Indiana Immunization Coalition has developed the capacity to go to schools and businesses to do immunization clinics for both adults and children including flu.
