APRA Request

I acknowledge that when I submit this request, the request will become a matter of public record, and my name and the content of my request will be posted on a public website.

I understand that if the records I’m requesting are not held by the La Porte County, Indiana Government or one of its agencies, my request will not be fulfilled. I understand that if my request is deemed a bulk data request, I will be prompted to complete a different form for requesting records. I understand that if my request is missing information necessary for fulfillment, I will be informed of the missing information, and I will have to resubmit my request with that information included in order for my request to be fulfilled. I understand that if I request copies of records to be mailed to me, a copying fee may be charged; I will be informed of the copying cost and will pay the cost prior to receiving any copies.
